WHAT IS ERECTILE DYSFUNCTION

What is Erectile Dysfunction? Why is it more than just “getting older”?

Erectile Dysfunction (ED) is defined as the persistent or recurrent inability to achieve or maintain a sufficient erection for satisfactory sexual activity, with symptoms lasting more than 3 months warranting medical evaluation. ED is not simply a “sexual problem” — it is often a Vascular health, hormonal levels, nerve conduction, and psychological well-being an early warning signal across four major body systems. In the Nanjing area, rising rates of workplace stress, hyperlipidemia, and hyperglycemia have led to a notable increase in ED prevalence among Asian men over 40.

Why Is ED Not “Simply a Psychological Problem”?

Clinically, more than 70% of ED cases presenting after age 40 involve An organic vascular or hormonal basis. Attributing all ED to “overthinking” risks missing the early warning window for cardiovascular disease.

Why is ED often an early warning sign of cardiovascular disease?

Penile arterial diameter is only 1–2 mm — far smaller than the coronary arteries.ED Appears on average 3–5 years before a cardiac event. Viewing ED as an opportunity for a complimentary cardiovascular health check is far more valuable than simply reaching for medication.

Why can’t we rely solely on PDE5 medications?

PDE5 inhibitors such as sildenafil and tadalafil address only the immediate erection — they do not treat the underlying cause.Long-term reliance alone can mask the underlying issue, vascular and hormonal deterioration continues.

Why is a complete assessment always the first step?

Blood testosterone, lipid panel, blood glucose, IIEF-5 score, and cardiovascular risk assessment are all essential — none can be skipped.Prescribing or injecting without assessment is the most common error seen in under-medicalized clinics.

Clinical Notes:Many clients come to us having been sold a “one-shot solution” elsewhere — with no blood testing or hormonal assessment. The results are often effective short-term but recurrent long-term. This is a textbook consequence of under-medicalized men’s intimate care. Our physician’s protocol The first step is always a medical evaluation, rather than proceeding directly to a treatment plan.

CAUSES

The Five Causes of Erectile Dysfunction — It’s Not Just “Age” or “Stress”

Clinically, purely psychogenic ED accounts for fewer than 30% of cases. Among patients over 40, the proportion with an organic etiology rises significantly. Most cases are Multifactorial overlap rather than a single cause.

1. Vascular (insufficient arterial blood supply)

Hypertension, hyperlipidemia, and atherosclerosis lead to insufficient cavernous blood flow. This is the Most common organic etiology, is also an early warning signal for cardiovascular events.

2. Endocrine (testosterone / thyroid / prolactin)

Low serum testosterone, subclinical hypothyroidism, and hyperprolactinemia all significantly impair libido and erectile function. In Asian men over 45,Testosterone naturally declines at approximately 1–2% per year, requires medical assessment.

3. Neurogenic (diabetes / surgery / neuropathy)

2 The prevalence of ED exceeds 50% among individuals who have had diabetes for 10 or more years. Prostate surgery, pelvic radiation therapy, and spinal cord injury can all cause neurological conduction impairment.

4. Drug-induced

Certain medications are associated with ED, including antidepressants (SSRIs), antihypertensives (beta-blockers, thiazide diuretics), and 5-alpha reductase inhibitors.Medication history is critical to the assessment.

5. Psychological support + lifestyle modification

Anxiety, depression, relationship tension, sleep deprivation, smoking, excessive alcohol use, obesity, and a sedentary lifestyle — these are common among working Asian men in Nanjing and interact with organic causes to compound the problem.

DIFFERENTIAL DIAGNOSIS

4 Types of men’s intimate conditions commonly confused with ED — key differential diagnosis points

Misclassification leads to misdirected treatment. The table below outlines the 4 most common differential diagnoses encountered in our physician’s clinical practice.

Low Libido / Hypoactive Sexual Desire Disorder (HSDD)

Hypoactive Sexual Desire Disorder

Key issues Lack of interest in sexual activity, rather than a problem with erection itself
Erectile function Erectile function is normal when interest is present
Primarily associated Low testosterone / depression / relationship fatigue
Triggering factors Primarily endocrine and psychological factors
First-line approach Hormonal assessment + psychological counseling + cautious use of TRT

Premature Ejaculation

PE · Unrelated to erectile quality

Key issues Ejaculatory timing control — not an erection concern
Erectile function Erection achieved but duration is short
Primarily associated 5-HT Neurotransmitters + anxiety
Triggering factors Often begins in adolescence or develops secondary to ED-related anxiety
First-line approach Behavioural therapy + topical anaesthesia + SSRI evaluation

Peyronie’s Disease

Peyronie’s disease / penile curvature

Key issues Fibrous plaques causing penile curvature and/or pain during erection
Erectile function May co-exist with ED, but the primary concern is appearance
Primarily associated Physical trauma + cumulative microtrauma
Triggering factors 40-70 commonly presents at this age; palpable fibrous plaque may be present
First-line approach Specialist assessment + injection / extracorporeal shockwave therapy

Male Andropause Syndrome

Late-Onset Hypogonadism · LOH

Key issues Systemic presentation of declining hormone levels
Erectile function ED is one of several presentations, often accompanied by fatigue, low mood, and reduced muscle mass
Primarily associated Systemic decline in testosterone
Triggering factors 45 and above, progressive
First-line approach Standardized TRT + lifestyle modification + long-term follow-up

⚠️ Important:In clinical practice, approximately 25% of clients who present with self-reported ED are reclassified after assessment as premature ejaculation or low libido — conditions that require entirely different treatment approaches. Treating every men’s intimate health concern as ED is the most common failure pattern seen in under-medicalized clinics.

ASIAN MALE FOCUS

Why does ED treatment for Asian men require Asian-specific clinical adjustments?

Common blind spots in mainstream North American andrology clinics

The parameters of mainstream North American andrology protocols are derived from large-scale clinical data in white male populations. Applying these directly to Asian men raises several key issues:

  • ✗ Deviation in testosterone reference ranges— The median baseline testosterone in Asian men is slightly lower; applying the standard North American lower reference limit may underestimate the problem
  • ✗ Dose overload— TRT Standard dosing in lighter-weight Asian patients frequently leads to erythrocytosis (excess red blood cells)
  • ✗ Shame-based cultural barriers go unrecognized— Asian men are more likely to self-conceal symptoms and delay seeking care; insufficient depth of clinical inquiry leads to missed diagnoses
  • ✗ Differences in body type and anatomical expectations— North American girth enhancement protocols may not align with the actual needs and tissue characteristics of Asian patients

Our physician’s Asian male-specific clinical adjustments

Parameters North American standard Asian-specific adjustment
TRT Starting dose Standard protocol Starting at a low dose, with reassessment at 8 weeks
Hemoglobin monitoring Every 6 months Intensive monitoring for the first 3 months, every 4 weeks
P-Shot Injection volume Uniform dosing Individualized distribution based on tissue characteristics
Consultation duration 15-20 minutes Initial consultation: 60 minutes, including psychological screening
Language environment English only Mandarin consultations with real-time interpreter devices · Private one-on-one

Key differentiators:Our physician specialises in treating Asian patients at our Nanjing clinic, serving a substantial number of Asian men seeking intimate health care. Our protocols are built on international andrology guidelines and refined to reflect Asian body composition, hormonal baselines, and cultural communication norms — not simply adapted from North American Caucasian male templates.

TREATMENT OPTIONS

6 Treatment Approaches for Erectile Dysfunction — Matched to Underlying Cause

There is no “magic shot” for ED. We design individualized combination plans based on your assessment findings — vascular, hormonal, neurogenic, psychogenic, or mixed etiology.

Recommended

Penile enhancement injection (hyaluronic acid / anatomical optimization)

Using highly cross-linked, medical-grade hyaluronic acid, precisely placed layer by layer for even distribution — improving penile girth and restoring confidence in appearance.Does not address erectile function directly, but can meaningfully improve self-image, making it particularly suitable for ED clients who also have concerns about anatomy. Performed by our physician personally.

Supportive

PDE5 Standardized inhibitor prescribing

Sildenafil, tadalafil, vardenafil, and other oral medications, when Clear contraindication exclusion(nitrate use, serious cardiovascular events, and glaucoma history) before prescribing, as a confidence-rebuilding tool during the initial phase of treatment — not as a long-term solution.

Supportive

NAD+ Anti-aging IV infusion

Systemic cellular energy support, offered as part of a comprehensive anti-aging protocol for ED clients over 45 who also present with overall energy decline and poor sleep quality. Not a standalone ED treatment.

Supportive

Lifestyle modification and cardiovascular co-management

Weight management, aerobic exercise prescription, smoking cessation, sleep optimization, and co-management with a family physician for hypertension, hyperlipidemia, or diabetes — without addressing the underlying cardiovascular foundation, any ED treatment remains a temporary measure.

TREATMENT FLOW

Our physician’s 6-Step Standard ED Protocol

01

Free 30-Minute Private Consultation

A private consultation room, Mandarin consultations with real-time interpreter devices, and a one-on-one physician consultation. Comprehensive intake includes IIEF-5 scoring, medical history, medication history, and cardiovascular risk factor assessment. No prescription without assessment.

02

Laboratory testing (blood panel + hormones)

Morning duplicate testosterone + free testosterone + SHBG + LH/FSH + prolactin + thyroid panel + lipid panel + fasting glucose + HbA1c + PSA (age 40+) + complete blood count. This panel forms the foundation for accurate subtype-based treatment planning.

03

Classification assessment + personalized treatment planning

Once results are available, our physician reviews and discusses them with the client:Vascular / Hormonal / Neurogenic / Psychogenic / Mixed, A combined protocol incorporating P-Shot® / TRT / PDE5 inhibitors / anatomical enhancement / cardiovascular co-management.

04

Active treatment phase (3–6 months)

P-Shot Typically 3 sessions, once every 4 weeks; TRT reviewed and adjusted every 8 weeks; PDE5 inhibitors used on demand. All injection procedures are performed by our physician personally, using medical-grade local anesthesia, in a fully private treatment room throughout.

05

Evaluation phase (4–6 weeks after the primary treatment course)

IIEF-5 Re-evaluation + repeat bloodwork + quantified patient-reported satisfaction. This assessment determines whether to transition to a maintenance phase or proceed with additional treatment.

06

Long-term maintenance phase

P-Shot Maintenance is typically once every 9–12 months; TRT requires a blood panel every 6 months; cardiovascular health, weight, and sleep are managed on an ongoing basis. ED is not a “fix it once” procedure — it is an entry point into long-term health management for men in their middle and later years.

TIMELINE

Treatment Timeline — How long before you see results?

Classification Active treatment cycle Initial results Meaningful improvement Maintenance cycle
Vascular P-Shot 3 sessions After the 1st–2nd session 4–8 weeks after the 3rd session Every 9–12 months
Hormonal TRT 3-6 months 4-6 weeks after 3 months later Long-term maintenance + six-month follow-up
Psychogenic / Mixed P-Shot 1-2 session(s) + psychological support 2-4 weeks 1-3 months As needed; typically no routine maintenance required
Refractory / diabetic P-Shot 3 sessions + TRT + multidisciplinary approach 2-3 months 6 months later Every 9–12 months on an ongoing basis

Clinical note:The biggest mistake in men’s intimate healthcare is expecting a “one-shot fix” before next week’s dinner. Responsible treatment takes time — vascular regeneration and hormonal stabilization are measured in months, not days. Among our Nanjing clients, those who complete a thorough assessment and follow a structured 6-month protocol consistently show significantly better long-term satisfaction and more stable outcomes.

PRICING PHILOSOPHY

About the Cost of Erectile Dysfunction Treatment

Pricing for men’s intimate health services varies widely across clinics — but the true cost is not about the device or the consumable. It reflects who performs the assessment, who determines the clinical direction, and how deeply physician expertise needs to be involved. ED is rarely a single-system problem: it may involve vascular, hormonal, neurological, psychological, and underlying cardiovascular factors. Cases that require deep physician involvement, laboratory-based classification, and long-term follow-up will carry a price structure that reflects that level of medical investment. For clients with a confirmed diagnosis who have entered a straightforward maintenance phase, certain steps may be carried out by trained medical team members under a physician-directed protocolmembers safely, making care more accessible. All injectable treatments are performed exclusively by the physician. The price difference in laser treatments fundamentally reflects the gap in clinical experience between physician-performed and technician-performed procedures. Which protocol is right for you is determined by your condition and assessment findings — not by price. The specific plan will be discussed together after your consultation and once laboratory results are available.

  • Payment is made at the clinic, after the consultation and before treatment begins
  • Nothing is paid in advance, and no deposit is required to hold an appointment
  • Cosmetic treatment is a self-pay item and is not covered by public medical insurance in China
  • All new clients receive a 30-minute Free Private Consultation and Assessment
RISKS & INFORMED CONSENT

Risks & Contraindications — Full transparency is essential

What are the main risks?

1. Temporary swelling and bruising at the injection site(common, resolving within 24–72 hours); 2. Rare hematoma or infection (uncommon under strict sterile technique); 3. TRT may cause erythrocytosis, mild edema, or acne — managed through blood monitoring and dose adjustment; 4. PDE5 inhibitor-related headache, facial flushing, and indigestion (common and transient); 5. Uneven distribution or lumpiness following girth enhancement injections (significantly reduced with proper layered injection technique); 6. Individual variation in treatment response — patients with severe diabetes or long-standing cardiovascular disease may respond more gradually.

Who is absolutely not a candidate?

Patients currently using nitrates are contraindicated for PDE5 inhibitors. Those with recent unstable angina, severe heart failure, or a heart attack or stroke within the past 6 months are contraindicated for all erection-enhancing protocols. Patients with active prostate or breast cancer that has not been evaluated are contraindicated for TRT. Patients with severe coagulation disorders that have not been addressed are contraindicated for all injections. Active local skin infection, severe unassessed allergy history, or unrealistic expectations (believing a single injection is a permanent cure) — in these situations, our physician will recommend addressing the underlying issue first or deferring treatment.

How are risks minimized during treatment?

Five key safety principles: (1) Complete laboratory testing — no skipping; (2) Start TRT at a low dose with intensive monitoring of red blood cell count and PSA for the first 3 months; (3) All injections performed by the physician personally, in a medical-grade sterile environment; (4) Avoid strenuous activity and alcohol for 48 hours post-procedure; (5) Contact the clinic and seek medical attention immediately if you experience persistent pain, an erection lasting more than 4 hours, or any unusual changes.

What Does the Informed Consent Form Include?

Every client is required to sign an informed consent form before their first treatment. It covers: how the treatment works, possible adverse reactions and their likelihood, alternative options, criteria for discontinuing treatment, follow-up scheduling, laboratory re-testing plans, and an optional Media Release. All content is explained in person by our physician — not signed off by administrative staff. Men’s intimate health consultations include dedicated private time specifically for questions.

Where the medical claims on this page come from

Everything above about Erectile Dysfunction (ED) rests on published evidence rather than on our own word. The sources below are the ones behind those statements: peer-reviewed papers indexed in PubMed, and guidance written for patients by dermatology academies and national health services. Read them before you decide — and bring any of them to your consultation.

They are not an assessment of this clinic, and no published study can tell you whether it suits your skin type, anatomy, or medical history. That judgement comes from the physician who examines you in person, and it may well be “not this one”.
