Masseter Hypertrophy
Masseter hypertrophy — commonly referred to as a “square face,” “rectangular face,” or “enlarged jaw angle” — occurs when the masseter.

What is masseter hypertrophy? And why is it different from simply having a “chubby face”?
Masseter hypertrophy — commonly referred to as a “square face,” “rectangular face,” or “enlarged jaw angle” — occurs when the masseter, the largest of the chewing muscles, becomes Genetic predisposition combined with chronic clenching or bruxism abnormal hypertrophy, giving the lower face a wide, square appearance. This is not a fat issue, nor a skeletal issue — it is Muscle concern, Weight loss, body sculpting, and dietary changes will therefore have no effect on this.
Why doesn’t weight loss help?
The masseter is muscle tissue — it does not shrink with fat loss. Many clients lose 10 pounds yet their face remains square, precisely because the underlying cause is Masseter thickness not the buccal fat pad.
Why is masseter hypertrophy more prevalent among Asian women?
East and Southeast Asian populations tend to have a wider jaw structure, a diet that includes harder-textured foods such as rice and noodle dishes, and a high prevalence of stress-related teeth grinding — all of which keep the masseter in a state of High-frequency active state.
Why Can’t Bone Reduction Surgery Solve This?
Jaw angle reduction surgery targets Skeletal morphology, In over 70% of square-face presentations, muscle — not bone — is the primary driver. A neurotoxin jaw-slimming assessment is often the right first step, and can help avoid unnecessary jaw reduction surgery.
Why is a bite test essential before treatment?
palpating the masseter at maximum occlusal contraction to assess thickness, symmetry, and whether jawline drooping is present, This determines the dosage distribution and injection points — it is never a one-size-fits-all “X units per side” approach.
Clinical Notes:Many clients have been told elsewhere that “jaw slimming injections will do the job” — and end up going through multiple treatment cycles — but no occlusal assessment has ever been performed, and no palpation-based classification of mixed-type cases. The first step in our physician’s protocol is always classification and palpation — never reaching for the needle straight away.
The five main causes of masseter hypertrophy — it’s not just genetics
Clinically, purely genetic masseter hypertrophy accounts for fewer than half of all cases. The majority are Genetic predisposition combined with acquired habits a cumulative result.
1. Genetic predisposition (jaw bone structure + baseline masseter size)
Individuals of East Asian descent typically have a smaller gonial angle and a broader masseter attachment area. Clients with a family history of a “square face” often present with a baseline masseter thickness exceeding 12–15 mm (compared to the normal range of 8–10 mm).
2. Bruxism (nighttime teeth grinding / jaw clenching)
Unconscious teeth grinding during sleep or daytime jaw clenching keeps the masseter in a state of sustained concentric contraction chronic overactivation. Over time, the muscle fibers hypertrophy in much the same way as they would with repetitive strength training — this is the primary reason the lower face can appear increasingly square-shaped after age 30.
3. Long-term chewing of hard or tough foods
Habitual chewing of gum, beef jerky, nuts, hard rice, or ice significantly stimulates masseter thickening. Clinical observations suggest that regular gum chewers may experience a 15–20% increase in masseter thickness within 6 months.
4. Psychological stress / anxiety
Anxious personality types tend to clench their jaw unconsciously under stress — which is why “work stress” and “jaw soreness / a progressively squarer face” so often go hand in hand.
5. Temporomandibular joint disorder (TMD)
TMJ joint imbalance forces the masseter to compensate with excessive workload, leading to hypertrophy. These cases require Co-assessment with a dentist or oral and maxillofacial surgeon, Jawline-slimming injections alone can improve appearance, but they do not address the underlying cause.
4 types of “wide face” that are easily confused with masseter hypertrophy — key differential diagnosis points
Misclassification leads to the wrong treatment. The table below outlines the 4 most common causes of perceived facial width that our physician encounters in clinical practice.
Buccal fat pad enlargement
Chubby Cheeks · Baby fat
| primary treatment area | Below the zygomatic arch, lateral to the corners of the mouth |
|---|---|
| Tactile feel | Soft, with no detectable muscle tension |
| changes upon occlusion | no change |
| typical candidates | 20-35 year-old female with a round face |
| first-line treatment | Buccal fat pad removal or lifting indicated — neurotoxin alone is ineffective |
Bony Wide Jaw
Mandibular Angle Hypertrophy
| primary treatment area | bony jaw angle prominence |
|---|---|
| Tactile feel | firm, inelastic |
| changes upon occlusion | no change |
| typical candidates | more common in males; familial / genetic predisposition |
| first-line treatment | maxillofacial surgical assessment (jaw angle reduction) |
Masseter Hypertrophy
Page topic
| primary treatment area | anterosuperior to the jaw angle, inferior to the earlobe |
|---|---|
| Tactile feel | Noticeably bulges and hardens during biting |
| changes upon occlusion | Visibly hypertrophied (key differential point) |
| typical candidates | Asian female, teeth grinding / stress-related clenching |
| first-line treatment | Botox / Dysport / Xeomin / Letybo |
Lower facial skin laxity / sagging
Jowls · jawline drooping / mandibular border laxity
| primary treatment area | lateral jawline + submental area |
|---|---|
| Tactile feel | soft, with skin redundancy |
| changes upon occlusion | no change |
| typical candidates | 40+ years old, with collagen loss |
| first-line treatment | Sofwave / Ultherapy Tightening + volumizing |
⚠️ Warning:Clinically, approximately 25% of clients who come in requesting jawline-slimming injections are found on palpation to be primarily affected by buccal fat pad prominence or skin laxity — for these clients, botulinum toxin alone is not only ineffective, but excessive masseter atrophy could actually Worsening of jawline sagging. Inaccurate diagnosis → treatment failure → repeated clinic visits: this is the most common frustration for Asian clients seeking jawline contouring.
Why does jawline slimming for Asian faces require an Asian Jawline Contouring Specialist Protocol?
Blind spots in mainstream North American clinics
Most MedSpa neurotoxin jaw-slimming protocols in North America are calibrated around Caucasian facial aesthetics — high nasal bridge, longer face shape, naturally narrower jaw. Applying these parameters directly to Asian faces creates two core problems:
- ✗ blindly excessive dosing— Asian masseter muscles typically have a broader attachment surface but denser muscle fibers, requiring precise multi-point placement rather than high-volume flooding
- ✗ Insufficient assessment of jawline support— Asian women tend to have lower skin elasticity — excessive dosing can easily lead to jawline laxity
- ✗ injection placement too superficial or too low— may spread to the zygomaticus muscle or the marginal mandibular nerve, resulting in smile asymmetry
Our physician’s Specialist Adjustments for Asian Faces
| parameters | North American standard | Asian-specific adjustment |
|---|---|---|
| Unilateral dosage | Fixed high dosage | Personalized low-to-moderate dosage |
| Number of injection points | 2-3 points | 3-5 Point distribution |
| depth | Consistent | Layered deep injection technique |
| palpation assessment | Often omitted | Mandatory bite palpation |
| Jawline assessment | None | skin elasticity and drooping risk must be assessed |
Key differentiators:Our physician has focused exclusively on Asian faces at our Nanjing clinic for over 8 years — more than 90% of our clients are of Asian descent.
6 Treatment Approaches for Masseter Hypertrophy — Selected by Classification
There is no single “best” neurotoxin for jaw slimming. We select the most appropriate brand and supporting protocol based on palpation findings, your individual physiology, and your prior injection history.
Botox®
Allergan the onabotulinumtoxinA with the most extensive global research data. Onset is typically 5–7 days, with results lasting 4–6 months. Best suited for Most typical cases of masseter hypertrophy. Our physician.
Dysport®
Galderma under which it is classified as abobotulinumtoxinA. It has a slightly wider diffusion range, making it suitable for Dense muscle mass requiring uniform softening clients. Faster onset (3–5 days), with a comparable duration of results.
Xeomin®
Merz A “clean” neurotoxin with a protein-free formulation. Suitable for Resistance developing after multiple prior injections(antibody formation) or clients with sensitive constitutions.

Our physician’s 6-Step Standard Protocol for Masseter Slimming
Consultation + bite palpation assessment
30 During your complimentary consultation, our physician palpates the masseter at maximum bite force to assess muscle thickness, symmetry, and fiber distribution, while also evaluating the buccal fat pad, jaw bone structure, and degree of skin laxity.no classification, no injection.
Classification + brand selection
Based on muscle thickness, injection history, and skin elasticity, we select the most appropriate product — Botox®, Dysport®, Xeomin®, or Letybo® — along with a customized dosage and injection-point distribution plan.
Safety triangle landmark placement + multi-point injection
3–5 precise deep injection points within the lower one-third safety triangle (Hinz Triangle) of the masseter, carefully avoiding the marginal mandibular nerve, risorius muscle, and parotid gland.Performed by our physician personally — never delegated.
4 First follow-up visit at week
Re-evaluate the degree of masseter softening, left-right symmetry, and the naturalness of your smile. If any localized compensatory bulging or asymmetry is present, a targeted touch-up is performed.
8 weeks for results to fully define
This is the optimal window for contour assessment. VISIA imaging and standardized photographs are used to compare before-and-after differences and determine whether the dosage for the next cycle needs to be adjusted.
Maintenance cycle (every 4–6 months)
With consistent maintenance over the first 2 years, and once muscle memory has stabilized, treatment intervals can be extended to every 8–10 months.Avoid overly frequent injection intervals— This is key to preventing jawline laxity.
Treatment Timeline — How Soon Will You See Results?
| phase | Timeline | Changes | recommendation |
|---|---|---|---|
| Early onset phase | Days 5–7 | Reduced masseter strength during maximum bite effort | Resume a normal diet; avoid chewing hard foods with excessive force |
| Softening phase | Weeks 2–4 | Palpation confirms masseter softening; initial narrowing of the jawline contour becomes visible | 4 first follow-up visit for fine-tuning |
| stabilization phase | Weeks 6–8 | The most defined “oval face” contour result | Standardized photographic documentation |
| Stabilization phase | Months 3–4 | Results are maintained while bite strength gradually recovers | prepare for next-cycle follow-up |
| regression phase | Month 5–6 | The muscle gradually recovers partial volume | schedule the next maintenance injection |
Clinical note:Rushing the process is the second most common mistake in masseter slimming — the first being injecting without a proper diagnosis. During the first 2–3 treatment cycles, muscle memory has not yet stabilized, and results will gradually last longer with each session. Among our clients in Nanjing, those who follow a consistent protocol for 2 years are often able to extend their intervals to every 8–10 months.

Pricing for Masseter Slimming Treatment
Pricing for masseter slimming varies widely across clinics — but the real cost driver is not the neurotoxin brand itself. It comes down to who performs the assessment, who determines the dosage distribution, and how much clinical expertise the case demands. Masseter hypertrophy is a complex condition to classify (pure muscle hypertrophy / mixed buccal fat pad involvement / prior over-injection revision), requiring palpation of the bite, asymmetry micro-adjustment, jawline ptosis risk assessment, and review of any antibody history. Cases of this complexity involve our physician directly, and the pricing reflects that level of physician-led care. All injectable treatments at our clinic are performed by the physician personally — never delegated to technicians. For clients who have established a regular maintenance routine and whose muscleFor returning clients whose muscle memory has stabilized, the treatment protocol is more standardized and pricing is more accessible. Laser and energy-based treatments may be performed by our trained clinical team under the physician’s protocol; however, botulinum toxin dosage assessment, injection depth placement, and symmetry fine-tuning, This expertise comes from years of accumulated clinical experience and refined anatomical feel. The right protocol for you is determined by your masseter classification and contour goals — not by price. Everything will be discussed in detail at your consultation, guided by hands-on palpation and your prior injection history.
- Payment is made at the clinic, after the consultation and before treatment begins
- Nothing is paid in advance, and no deposit is required to hold an appointment
- Cosmetic treatment is a self-pay item and is not covered by public medical insurance in China

Risks & Contraindications — What You Need to Know
What are the main risks?
1. Reduced chewing strength(common; resolves naturally within 2–4 weeks); 2. Bruising / swelling at the injection site (common; resolves within 3–7 days); 3. Left-right asymmetry (rare; correctable with a minor adjustment within 4 weeks); 4. Stiffness of smile / asymmetry of mouth corners (extremely rare; caused by spread to the zygomaticus muscle, typically resolves naturally within 4–6 weeks); 5. Jawline ptosis caused by over-injection(risk is higher with frequent high-dose treatment); 6. Transient headache / localized soreness (1–3 days).
Who is absolutely not a candidate?
Pregnancy, breastfeeding, myasthenia gravis / Lambert-Eaton syndrome, severe allergy to neurotoxins or egg / milk protein, active infection at the injection site, bleeding disorders or ongoing anticoagulant use, serious neuromuscular disorders, and unrealistic expectations (believing a single treatment produces permanent results) — in any of these situations, our physician will recommend postponing treatment or exploring alternative options.
How Is Jawline Laxity Risk Minimized During Treatment?
Our 5 core principles: (1) Hands-on consultation with mandatory bite palpation and skin laxity assessment before every treatment; (2) Start at the lower end of the dosage range and titrate upward based on response; (3) Maintain a minimum interval of 4 months between sessions; (4) After 2 years of consistent treatment, proactively extend intervals to every 6–8 months; (5) For clients over 30 with reduced skin elasticity, co-manage with a Sofwave skin-tightening protocol.
What does the informed consent form include?
Every client signs an informed consent form prior to their first injection. It covers: how botulinum toxin works, potential adverse reactions and their likelihood, alternative treatment options, criteria for discontinuing treatment, follow-up arrangements, and an optional media release. All content is explained in person by our physician — not delegated to administrative staff.
