What Is Facial & Neck Laxity — and Why Is It More Than Just a “Skin Problem”?
Facial & Neck Laxity refers to the age-related changes occurring across multiple tissue layers of the face and neck, including descent, volume loss, and reduced elasticity a complex, multi-layered process. It is far more than simply “loose skin” — it involves skin, SMAS fascia, fat pads, skeletal support, and platysma the result of all five layers aging simultaneously. This is precisely why single-modality, single-depth treatments rarely resolve overall facial descent.
Why Is Surface Skin Tightening Alone Not Enough?
surface-level skin tightening (such as basic radiofrequency) only acts on the superficial dermis, and has little effect on SMAS Fascial descent completely ineffective. Why Does Facial Laxity Present Differently in Asian Faces?

Five Key Causes of Facial and Neck Laxity — It’s Not Simply “Aging”
Two patients of the same age can differ by as much as a decade in visible laxity. The reason is never simply time — it is the cumulative effect of the five factors below.
1. SMAS Fascial laxity and descent
SMAS(the superficial musculoaponeurotic system) is the structural “foundation garment” of the face. After age 35, the SMAS loses elasticity and begins to descend — causing the midface to drop, nasolabial folds to deepen, and the jawline to lose definition. This is the target layer for both surgical facelifts and Ultherapy®.
2. Progressive loss of collagen and elastin
25 after a certain age, collagen loss occurs at approximately 1% per year, accelerating to around 2% around menopause. As the skin thins and loses elasticity, fine lines and superficial laxity emerge under the effects of gravity.
3. volume loss (skeletal resorption + fat atrophy)
The zygomatic and mandibular bones undergo gradual resorption with age, deep fat pads atrophy, and superficial fat pads descend. 4. Platysma laxity
After age 40, the anterior platysma bands loosen and form vertical cords (commonly called “turkey neck”), while the posterior bands sag and blur the jawline. Addressing neck laxity Muscle dynamics must be addressed concurrently, skin tightening alone is ineffective.
5. Photoaging & Lifestyle Factors
Prolonged UV exposure degrades dermal elastic fibers — photoaging is the single greatest accelerator of skin laxity. Smoking, rapid weight loss, poor sleep, and glycation also significantly accelerate the process.

Why Does Treating Laxity in Asian Faces Require an Asian-Specific Protocol?
A blind spot in most North American clinics
Laxity in Caucasian patients tends to present as Periorbital drooping + deep neck lines, treatment strategies tend to favor aggressive, overall lifting. However, the pattern of laxity in Asian faces is fundamentally different:

Primary treatment phase (single session or staged)
Energy-based treatments (Ultherapy® / Thermage® FLX) are typically completed in a single session; Sofwave™ may be layered over 1–2 sessions. Thread lifting and injectables can be performed concurrently or staggered 2 weeks apart, depending on the plan. 3 Month outcome assessment window

What are the main risks associated with energy-based anti-aging treatments?
1. 1. Discomfort during treatment (more noticeable during deep Ultherapy® targeting; topical anesthesia and oral analgesics are available); 2. Temporary redness and swelling (common, resolving within 24–72 hours); 3. Occasional superficial burns or blistering (rare under our physician’s protocol, typically associated with excessive energy or incorrect targeting); 4. Transient sensory nerve changes (mandibular branch area, usually resolving within 4–8 weeks; extremely rare); 5. Results below expectation (most common in volume-loss patients, requiring re-evaluation).
Who Is Absolutely Not a Candidate?
Pregnancy, breastfeeding, facial implants or metal wires, cardiac pacemakers (contraindicated for certain radiofrequency devices), active facial infection or dermatitis, severe keloid tendency that has not been assessed, active autoimmune disease, or unrealistic expectations (believing one treatment can take 20 years off) — in these situations, our physician will recommend deferring treatment or selecting an alternative approach.

Thread lifts: swelling for 24–72 hours, mild bruising, and rare thread migration or surface irregularities — risks significantly reduced when a physician personally controls placement and tension. Injectables: bruising, injection-site discomfort, and extremely rare but serious vascular events — which is precisely why we insist that all injectable treatments must be performed by the physician personally the core reason why a physician must be present — to immediately recognize and manage any vascular signs.
What Does the Informed Consent Form Include?
