The 5 Causes of Facial Sagging and Jowls — It’s More Than Just “Aging”
Clinically, sagging is never caused by a single factor. We must distinguish primary from secondary causes to set the correct treatment sequence.
1. Collagen loss
25 After this age, skin collagen is lost at roughly 1% per year; after 40, dermal collagen density declines markedly, skin elasticity decreases, and fine lines and laxity appear. This is the most superficial layer of degeneration.
2. SMAS fascia descent
SMAS(Superficial Musculoaponeurotic System) is the “internal sling” that supports facial soft tissue. With age it stretches and loosens, loses tension, and drags cheek fat downward toward the jawline — this is the core mechanism behind jowls.
3. Fat pad displacement and atrophy
The face contains several independent fat compartments (buccal, malar, sub-malar, etc.). Midface compartments atrophy while lower-face fat accumulates, creating the classic “hollow above, heavy below” aging structure.
4. Bone resorption
40 After this age, the mandible, zygomatic bone, and orbital bone begin to resorb and thin. The facial “foundation” shrinks, and the overlying soft tissue loses its support and descends. This is the most difficult layer to address with non-surgical aesthetics — it requires volume replacement to simulate bony support.
