WHAT IS FACIAL FAT

What Is Facial Fat Accumulation? Why Is It Different from Being “Overweight”?

Facial Fat Reduction & Contouring What we are addressing is not simply a “weight problem” — it is contour blurring caused by irregular distribution within specific facial fat compartments. Because Asian faces tend to have a relatively flat skeletal framework combined with naturally fuller mid- and lower-face fat compartments, issues such as a rounded “chubby” face, a double chin, and an undefined jawline are common even when BMI is within the normal range. This is a medical aesthetic concern,Facial fat distribution management, This is not weight loss.

Why does our face still look full even after losing weight?

The buccal fat pad and submental fat are Discrete fat compartment, These fat deposits respond very poorly to diet and exercise. Many patients who lose body weight find their facial fat remains essentially unchanged — this is one of the most common concerns among Asian patients.

Why Is This More Pronounced in Asian Faces?

Asian faces tend to have lower cheekbones, a more inwardly set jaw angle, and naturally fuller midface fat pads. Combined with differences in how facial fat interacts with retaining ligaments, Asian faces often appear rounder than Caucasian faces at the same BMI.

Why is a one-size-fits-all surgical approach (bone shaving / liposuction) not appropriate?

Surgical options carry lengthy recovery times, higher risks, and irreversible outcomes. Modern non-surgical aesthetic medicine can Precise fat pad targeting A gradual, staged approach preserves facial support structures and youthful volume, preventing a hollowed or prematurely aged appearance.

Why Must Fat vs. Laxity Be Assessed First?

30 Jawline blurring after a certain age is often caused by Fat descent + skin laxity + bone resorption A mixed concern. Fat reduction alone will unmask laxity; skin tightening alone cannot address excess fat. A combination protocol is essential.

Clinical note:Facial fat is not a case of “less is more.” After age 35, fat pads begin to atrophy and descend, and over-reducing facial fat can accelerate an aged appearance. Our physician’s protocol Step one is assessing fat distribution and age-related facial trajectory, Precisely addressing areas of excess while preserving structural support zones.

CAUSES

Five causes of excess facial fat — it’s not just about weight

Clinically, weight-related facial fat accounts for only a small proportion of cases. Most clients’ facial fat concerns are Structural factors + genetics + age combined.

1. Genetically full fat pads

The buccal fat pads and submental fat compartments in Asian faces are genetically 20–30% larger than in Caucasian faces. This has nothing to do with body weight — it is a structural, inherited characteristic.

2. Mandibular developmental retrusion

The relatively inwardly set jaw angle and underprojected chin common in Asian bone structure can create the appearance of a less defined jawline and a fuller submental area — even when fat distribution is normal.

3. Age-related descent of fat pads

35 After this age, the malar fat pad descends and accumulates along the jawline, creating jowling and a sagging appearance — this is fat redistribution, not fat gain.

4. Localized fat retention following weight fluctuation

After weight loss, overall body fat decreases, but facial fat pads often shrink disproportionately little — leading to the common complaint of “Our body is slimmer but our face still looks full.” Targeted, precise treatment is needed.

5. Edema / impaired lymphatic drainage

Some cases of facial puffiness are not true fat accumulation — they stem from poor lymphatic drainage and chronic low-grade inflammation. These conditions must be identified and addressed before any fat reduction treatment is considered.

DIFFERENTIAL DIAGNOSIS

4 Conditions Commonly Mistaken for Facial Fat — Key Differential Diagnosis Points

Misclassification leads to the wrong treatment. The table below outlines the 4 most common differential diagnoses our physician encounters in clinical practice.

Facial Edema

Lymphatic Stasis · Lymphatic retention

Morphology Heavier in the morning, lighter in the evening / pitting on pressure
Palpation Soft to the touch, no palpable fat nodules
Trigger Sleep / sodium intake / menstrual cycle / allergy
Durability 1 noticeable change within days
First-line treatment Lymphatic drainage + lifestyle modifications · Not a fat reduction treatment

Skin Laxity

Skin Laxity · Dermal / SMAS laxity

Morphology Jawline overflow + early jowl formation
Palpation Soft skin with poor elasticity
Trigger Age / photoaging / rapid weight loss
Durability Gradual progression
First-line treatment Sofwave / Ultherapy Prioritize tightening; proceed with fat reduction cautiously

Masseter Hypertrophy

Masseter Hypertrophy · Masseter hypertrophy

Morphology Square mandibular angle with posterior fullness
Palpation Palpable firmness when clenching the jaw
Trigger Bruxism / chewing habits / genetics
Durability Stable, no fluctuation
First-line treatment Botox® masseter injection · Not a fat-dissolving treatment

Skeletal square jawline

Mandibular Angle Prominence

Morphology Bony flaring of the jaw angle
Palpation Bony firmness on palpation · No fat or muscle component
Trigger Skeletal development
Durability Long-term stable results
First-line treatment Surgical evaluation may be warranted · Outside the scope of medical aesthetics

⚠️ Warning:Clinically, 25% of clients who present requesting facial fat reduction are reassessed and found to have masseter hypertrophy, skin laxity, or simple edema instead. Treating these conditions with Belkyra® or CoolSculpting® is not only ineffective — it can actively worsen their appearance.

ASIAN FACE FOCUS

Why does fat management for Asian faces require a specialized assessment approach?

Blind spots in mainstream North American protocols

Most medical aesthetic clinics in North America design their fat reduction protocols around the anatomical baseline of Caucasian faces, which have stronger skeletal support and relatively less prominent buccal fat. Applying these protocols directly to Asian faces creates three distinct problems:

  • ✗ Over-reduction of cheek fat— Weaker cheek skeletal support in Asian faces means excessive fat reduction accelerates hollowing and aging after age 35
  • ✗ Over-augmentation of the jawline— Blindly applying Caucasian “square jaw aesthetics” and ignoring the softer facial lines characteristic of Asian faces
  • ✗ Underestimating the proportion of submental fat— The submental fat compartment in Asian faces sits deeper, requiring a more precise injection protocol

Our physician’s Protocol Adjustments for Asian Facial Fat Reduction

Parameters North American standard Asian-specific adjustment
Buccal fat pad treatment Overly aggressive excision or cryolipolysis Conservatively retain 30–50%
Submental grid Wide grid spacing Precise grid mapping + superficial-layer targeting
Jawline aesthetics Square, prominent mandibular angle Soft V-line / oval face shape
Combined tightening Optional 35+ Essential
Age trajectory assessment Infrequently performed Mandatory 5–10 year age trajectory forecasting

Key differentiating factors:Our physician has specialized in Asian faces in Nanjing for over 8 years, with more than 90% of our physician's patients being of Asian descent.

TREATMENT FLOW

Our physician’s 6-Step Standard Protocol for Facial Fat Reduction & Contouring

01

Consultation + Classification: Fat / Laxity / Muscle / Skeletal

30 Free consultation includes: manual palpation to assess fat pad location, VECTRA 3D imaging, skin elasticity testing, and dynamic masseter evaluation. No fat reduction treatment is initiated without a complete skin and structural classification.

02

Aligning Aging Trajectory with Aesthetic Goals

Our physician discusses a 5–10 year facial aging projection with each client to avoid the outcome of “slimmer now, but looking older at 40.” The goal is to precisely address areas of excess while preserving the structural fat that supports a youthful appearance.

03

Customized device and injectable protocol

Based on treatment area, skin elasticity, and age, our physician selects Belkyra®, CoolSculpting, Emsculpt NEO®, or lipolytic injections — used alone or in combination — with a skin-tightening module built into the plan from the outset.

04

Active Treatment Phase (2–4 sessions, once every 6–8 weeks)

All injectable treatments are performed by our physician personally. Device-based treatments are carried out by the clinical team under our physician's prescribed protocol. Immediately following each session: cold compress, compression mask, and lymphatic drainage guidance are provided.

05

Assessment Period (8–12 weeks after primary treatment course)

VECTRA 3D Re-assessment with quantitative 45° lateral cervicomental angle comparison. Determines whether additional treatment is indicated or whether the client transitions to the maintenance phase.

06

Maintenance Phase (long-term; varies by protocol)

Belkyra Fat reduction results No routine maintenance required(Fat cells do not regenerate once destroyed); however, the skin-tightening and masseter Botox® components typically require maintenance every 6–18 months.

TIMELINE

Treatment Timeline — How Long Before You See Results?

Plan Active treatment cycle Initial results visible Significant improvement Maintenance cycle
Belkyra Submental 2–4 sessions After the 2nd session 8 weeks after the 3rd session No routine maintenance required
CoolSculpting Mini 1–2 sessions 4–6 weeks 8–12 weeks No routine maintenance required
Emsculpt Neo Submental 4–6 sessions After the 3rd session 4 weeks after the 6th session Once every 6–12 months
Radiofrequency / ultrasound skin tightening 1–3 sessions Immediate 3 Collagen remodeling after months Once every 12–18 months

Clinical Note:Faster is not better when it comes to facial fat reduction. Our physician’s protocol is deliberately paced — each session includes a reassessment before the next energy level or dosage is determined. Among clients in Nanjing, those who followed the gradual, staged approach reported significantly higher satisfaction than those who pursued aggressive single-session treatments.

PRICING PHILOSOPHY

Pricing for Facial Contouring & Fat Reduction

Pricing for facial contouring and fat reduction varies widely across clinics — but the real cost driver is not the device itself. It reflects who assesses you, who interprets the interplay between fat, skin laxity, muscle, and bone, and how deeply clinical judgment is required. Facial fat reduction involves fat pad classification, age trajectory forecasting, injection grid mapping, and coordinated skin tightening — decisions in which our physician is personally and deeply involved. That level of medical expertise is reflected in our pricing structure. For all injectable treatments (Belkyra / fat-dissolving injections), we require the physician to perform the procedure personally, because the submental region of the face involves the marginal mandibular branch of the facial nerve— among other structures — where hands-on experience directly determines both safety and outcome. For clients in a stable maintenance phase using device-only protocols (e.g., cryolipolysis, skin-tightening synergy), a trained clinical team can safely perform treatments under a physician-directed plan, making these options more accessible. The price difference between laser and energy-based treatments fundamentally reflects the gap in clinical experience between physician-performed and technician-performed (physician vs. technician) procedures. The right protocol for you is determined by your clinical presentation, not by price — the specific arrangement will be discussed at your consultation based on physical assessment and VECTRA 3D evaluation.

  • Payment is made at the clinic, after the consultation and before treatment begins
  • Nothing is paid in advance, and no deposit is required to hold an appointment
  • Cosmetic treatment is a self-pay item and is not covered by public medical insurance in China
  • All new clients receive a complimentary 30-minute Free Consultation & Assessment
RISKS & INFORMED CONSENT

Risks & Contraindications — What You Need to Know

Belkyra What are the primary risks of injectable treatments?

1. Swelling(Seen in almost all clients, 4–7 days) is a normal sign that treatment is taking effect; 2. Bruising(Moderate, 1–2 weeks); 3. Temporary marginal mandibular branch weakness(Rare < 2%, Typically resolves fully within 4–8 weeks); 4. Numbness / altered sensation(< 4 weeks); 5. Nodule(rare, and directly related to injection depth — which is why physician-performed treatment matters); 6. Asymmetry(Can be refined in a subsequent session).

CoolSculpting main risks?

Common: a pulling sensation during treatment, numbness lasting 1–4 hours, mild redness or bruising, and soreness or tightness for 1–3 weeks post-procedure. Rare: delayed sensitivity, PAH (paradoxical adipose hyperplasia — extremely rare in published reports < 0.1%). Data on paradoxical adipose hyperplasia (PAH) in Asian faces remains limited; therefore, our clinical protocol takes a conservative approach to cheek treatments, prioritizing the CoolMini applicator for precise, small-area applications.

Who should absolutely not undergo facial fat reduction?

Pregnancy, breastfeeding, significant coagulation disorders or ongoing anticoagulant use, active facial infection, history of facial nerve injury, severe keloid or hypertrophic scarring that has not been assessed, unrealistic expectations (e.g., believing one session will fully slim the face), or severe skin laxity that has not been addressed first — in any of these situations, our physician will recommend postponing treatment or resolving the underlying concern before proceeding.

What does the informed consent form include?

Before any first treatment, every client signs an informed consent form covering: the mechanism of action (Belkyra® disrupting fat cell membranes / cryolipolysis-induced apoptosis), potential adverse reactions and their likelihood, alternative options, criteria for discontinuing treatment, follow-up arrangements, and a Media Release (optional). All content is explained in person by our physician — not delegated to administrative staff.
