What is Xeomin® — the pure neuromodulator?

Xeomin® (is a product of Merz Pharmaceuticals, Germany, A botulinum toxin type A (incobotulinumtoxinA) formulation, NMPA-registered in 2010 for the aesthetic treatment of glabellar lines, and is The world’s third-largest neuromodulator brand. Its core differentiator is Proprietary XTRACT Technology™ purification process— removing all complexing proteins (accessory proteins), retaining only the active 150 kDa Neurotoxin molecule, also known as the “naked molecule.”

Botox and Dysport®, the toxin molecule is bound to 900 kDa Complexing proteins (hemagglutinins + non-hemagglutinin proteins) — these complexing proteins contribute nothing to treatment efficacy no direct contribution, however the primary antigenic source recognized by the immune system. After repeated long-term injections, some patients develop antibodies targeting the complexing proteins neutralizing antibodies, leading to diminished Botox® efficacy or complete loss of response (treatment failure / Secondary Treatment Failure).

Pre-procedure preparation

  • Avoid aspirin, ibuprofen, and Omega-3 supplements for 1 week before injection (to minimize bruising)
  • Avoid alcohol (for 24 hours prior to injection)
  • Disclose all current medications (including antibiotics and antihypertensives)
  • Makeup and eating are permitted as normal on the day of injection.
  • A complete neurotoxin history is required — including brand, dosage, and any history of diminishing or shortened effect.
  • Please inform us of any human serum albumin allergy (one of Xeomin®’s only excipients).

During the injection

  • Cleanse the face + mark muscle injection points (3–5 minutes)
  • Administered via 30–32G fine needles using a multi-point superficial injection technique

Common reactions (may occur in the short term following injection)
  • Mild redness and swelling / small raised bumps at injection sites (30 minutes to 2 hours to resolve)
  • Bruising (5-7 days to resolve naturally; Arnica may be used to accelerate recovery)
  • Sensation of tightness or heaviness in the head (D1-D5 common, typically subsides with time)
  • Mild headache (24-48 hours; acetaminophen may be taken for relief)

  • Unnatural smile— diffusion into the zygomaticus major during crow’s feet injections; Our physician avoids this by maintaining a distance of 1.5 cm+ from the lateral canthus
  • allergic reaction— Extremely rare (Xeomin®’s excipients contain only human serum albumin and sucrose); patients with known allergies should inform their provider.
  • Antibody formation (treatment failure)— Xeomin Antibody risk is already the lowest among the four major brands, but long-term high-frequency injections still require a minimum 12-week interval to reduce risk.
  • Infection at the injection site— Extremely rare; preventable with proper sterile technique.
