What Are Spider Veins? Why Do Leg and Facial Presentations Differ?

Spider veins (medical term: telangiectasia) are superficial dilated blood vessels typically less than 1–3 mm in diameter, named for their web-like or radiating thread appearance. On Face typically presenting as red vessels, commonly seen on the nasal alae and cheeks; on Legs They commonly appear blue-purple or red and are most often found on the thighs, behind the knees, and the lower legs. The two conditions have distinct underlying mechanisms and require entirely different treatment approaches — which is precisely why a differentiated assessment is essential.

Facial spider veins = fine red threads

Typically UV photoaging, rosacea, prolonged topical corticosteroid use causing superficial dermal arteriole and venule dilation, the hemoglobin presents as bright red. Vbeam® 595 nm is the preferred treatment.

Leg spider veins = blue-purple reticular pattern

Often associated with Venous valve insufficiency, prolonged standing, pregnancy, body weight, and genetics associated with elevated venous return pressure, the vessels tend to be deeper, larger, and blue in color. Sclerotherapy injection or Nd: YAG 1064 nm is typically required.

Why can’t a single device treat all types of spider veins?

Facial red vessels are superficial and fine → 595nm pulsed dye laser. Leg vessels are deeper and under higher pressure → 1064nm Nd: YAG or sclerotherapy. A mismatch in wavelength or mechanism will yield no results — or make things worse.

CAUSES

Facial and leg spider veins may look alike, but their underlying drivers differ considerably. Understanding the mechanism is essential for precise treatment and preventing recurrence.
