WHAT ARE SPIDER VEINS

What are spider veins, and why are they different from ordinary facial redness?

Spider veins (medical name: telangiectasia) refer to superficial skin vessels measuring 0.1–1mm in diameter,Permanent dilation of capillaries / small veins, appearing red, purple, or blue, resembling spider webs, fine threads, or branching patterns. They can occur on Face (nasal sidewalls, cheeks, jawline), and can also occur on Legs (outer thigh, calf, ankle). The two areas differ completely in cause, device choice, and protocol.

Why isn’t it just “red capillaries” and done?

“Red veins” is a colloquial term that may refer to simple diffuse facial redness, spider veins, or rosacea Rosacea early presentation. The three have different treatment plans — misdiagnosis means failed treatment.

Why is IPL not the first choice?

IPL is broad-spectrum light with low vascular selectivity, which on Fitzpatrick III–V Asian skin can easily overload and cause Epidermal burn + post-inflammatory hyperpigmentation (PIH). Vbeam 595nm matches the hemoglobin absorption peak, with selectivity far higher than IPL.

Will spider veins go away on their own?

No. Once capillaries dilate to a visible degree, the vessel wall structure is permanently altered — they must be cleared via laser coagulation or sclerosant occlusion. Waiting will only allow the area to expand.

Clinical pearl:Clinically, many clients have been told elsewhere that “it’s just ordinary redness — acid exfoliation / IPL is enough,” only to see repeated failures or worsening. Our physician’s protocol The first step is always diagnostic classification — distinguishing spider veins, diffuse erythema, and rosacea, rather than simply firing up the device.

CAUSES

Five causes of spider veins — distinct mechanisms for face and legs

Facial and leg spider veins have markedly different underlying mechanisms. Understanding the cause is essential to designing a personalized protocol and daily management plan.

1. Long-term UV accumulation (primary cause on face)

UVA Chronic UV exposure damages the elastic fibers of dermal vessel walls, causing permanent capillary dilation. High-latitude UV in North America combined with snow reflection accelerates this. This is The leading cause of facial spider veins.

2. Elevated venous pressure (primary cause on legs)

Prolonged standing/sitting, pregnancy, obesity, and chronic venous insufficiency (CVI) raise venous return pressure in the lower limbs, causing superficial small veins to dilate into spider-web patterns.The leading cause of leg spider veins.

3. Hormonal fluctuation

Pregnancy, oral contraceptives, and HRT (hormone replacement therapy) all increase vascular fragility. This is why spider veins are significantly more common in women than men.

4. Genetic predisposition / congenitally thin vessel walls

Those with a positive family history (parents with obvious leg varicose veins or facial redness) have significantly higher incidence. Thin Asian female skin more easily reveals superficial vessels.

5. Compromised barrier / chronic inflammation

Long-term corticosteroid use, excessive acid exfoliation, improper laser/light treatments, or chronic rosacea inflammation can cause facial capillaries to repeatedly dilate and eventually become fixed.

⚠️ Warning:Clinically, about 25% of clients who come in to “treat leg red veins” are determined after palpation to actually have Varicose veins / chronic venous insufficiency, Referral to vascular medicine is required first — sclerotherapy on these patients is not only ineffective, it will recur. Inaccurate diagnosis → treatment failure → repeated visits — this is the biggest pain point for leg vein patients.

Why do spider veins in Asian skin require a “Fitzpatrick III–V specialized protocol”?

Blind spots of mainstream North American clinics

Most North American aesthetic clinics base their vascular laser protocols on clinical data from Fitzpatrick I–II (Caucasian) skin. Applying these directly to Asian Fitzpatrick III–V skin creates three core problems:

  • ✗ Energy overload, high PIH risk— Epidermal melanin content is 5–10× higher, absorbing excessive heat

What are the main risks?

1. Bruising / purpura(Vbeam common afterward, resolves naturally in 1–2 weeks; significantly reduced with long-pulse protocol); 2. PIH Post-inflammatory hyperpigmentation(moderate risk in Asian skin, significantly reduced with low energy + test spot); 3. Transient erythema/dryness (common, resolves in 3–7 days); 4. Post-sclerotherapy Pigmentation at injection sites(common, resolves within 3–6 months); 5. Rare: blistering / crusting / epidermal necrosis (rare with proper protocol); 6. Spider vein recurrence (cannot be prevented, only managed).

Who absolutely cannot have this treatment?

Pregnancy, breastfeeding, oral isotretinoin within the past 6 months, unhealed sun exposure or sunburn within the past 4 weeks, active facial inflammation, severe keloid tendency not yet assessed, photosensitive disorders, history of deep vein thrombosis (leg clients), unscreened varicose veins / deep vein insufficiency, unrealistic expectations (believing 1 session can fully cure) — in these cases our physician will recommend postponement or referral for evaluation first.
